Request support for an Individual child from the Early Years and Childcare Service (EYCS)

Where a child is identified as having special educational needs, support should take the form of a four-part “Assess, Plan, Do, Review” cycle; where actions are tried, reviewed and revised as necessary in line with the child’s needs. In education settings, this is known as the graduated approach. Further information is available: Suffolk Graduated Response

If you have implemented stage 1 of the graduated response and you have moved to stage 2 and identified the child is making less than expected progress, you can seek support from the Early Years and Childcare Service if required. To seek support, providers must complete the referral form: request support for an individual child.

Parental Consent Form – Please be aware you will need to have gained parental consent before making a request to our service. Note: you do not need to send us this form, but it must be available if requested by a member of the EYCS.

Education, Health and Care (EHC) Needs Assessments

An Education, Health and Care (EHC) needs assessment can be requested by parents, carers, teachers or any other professional involved with a child or young person; they can also be requested by a young person over the age of 16.

You should use the EHC portal to complete your application – it allows you to log in and see the progress of your application at any time, provides secure messaging directly with your child’s named Family Services officer and helps us respond to requests and queries as quickly and efficiently as possible.
