Once you have opened this link scroll down to the section on the left hand side of the page, click “Paediatric Speech and language therapy” for information and guidance.

Go to the section on the on the left hand side and click “Referral information for professionals.”
On this page you will see the ICPS Referral form, please make sure you tick the speech and language box on the ICPS referral form as the service you require. Please note: this form should be returned to the NHS email address found on the top of the form itself.
