Please be reminded that all phase transfer annual review paperwork for students with EHC Plans who will be transferring between phases of education must be done and submitted to the Local Authority by October half term week (end of November for students moving into post-16 placements).
Invitations to the review meeting and requests for parent/child and any other views should therefore be issued ASAP.
This is to enable the LA to co-ordinate consultations and allocate places prior to the national deadlines of 15 February (31 March for post-16 transfers), but most importantly to enable children with SEND and their families to know their ongoing placements in good time, so that transition planning can commence.
Over the coming week, you will be contacted by the Family Services Officer linked to your school, who will be able to answer any questions you may have.
