A child or young person with an Education, Health and Care Plans (EHCP) who is transferring to a different phase of education in September the following year, must have an Annual Review, so that EHCP can be amended to name a placement by specific dates.
A Phase Transfer is a transfer between:
- Early years to school
- Primary school to middle school
- Infant to Junior School
- Junior to Secondary School
- Middle to secondary school
- Secondary school to a post-16 institution.
This date is the 15 February in all cases apart from secondary school to a post-16 institution which is the 31 March.
It is expected that the majority of requests for specialist provision will be within the Phased Transfer years and to support these Statutory Deadlines the following Annual Reviews must be completed and sent to the appropriate local Family Service Annual Review Mail box by:

However, if you are supporting a child who may need specialist in the next academic year then an Annual Review must be completed and sent by:
Non-Phase Transfer pupils who may require specialist provision from next academic year | September – November of current academic year. Deadline for submitting paperwork: 30 November |
All other year groups | Annually unless there are significant changes to the child/young person’s needs and the provision they require. |
